History Report

You have two options when sending us an application form, you can either fill out the form and pay online, or you can download the application form and post it to us with your payment. Please give as much relevant details as possible. If you have additional information that you feel is relevant, and will not fit on the application form, please include it on a separate sheet. Once you have completed the application form, please submit it to us, either online or by post.

Once an assessment is completed we will contact you with the results, this normally takes between 6 to 8 weeks, but at peak times of the year may take a little longer.
